Whether you’re facing cardiac, orthopedic or pulmonary issues, our dedicated subacute unit offers cutting-edge post acute care and rehabilitation services with therapies delivered daily. Our multidisciplinary team of professionals delivers the highest level of care with the overarching goal of getting you home as quickly and safely as possible. And once you’re back home, you can take advantage of our outpatient therapy services if the need ever arises.

Our long term care program caters to individuals with progressive illnesses and complex medical needs who require 24/7 skilled nursing care on a long term basis. We have a separate, secure wing for residents with memory impairment, that features its own activities program structured to meet the unique needs of this population.

A Proud History

In 1879, the Bradford County Commissioners purchased the former McKean Farm, consisting of approximately 268 acres, in Burlington, PA for the purpose of providing shelter for the indigent. Building construction was completed in 1880.

In 1963, the building was renamed the Bradford County Manor with the addition of a west wing. The east wing was added in 1964, and in 1985, major renovations were completed and two new wings added. Cosmetic upgrades and remodels were made over the years, including a state-of-the-art rehabilitation therapy gym.

Today, under the new ownership of Allaire Health Services, the facility was renamed Bradford Hills Nursing & Rehabilitation Center. Our 200-bed facility offers both private and semi-private rooms with 25 private short term stay rehabilitation suites, a secure memory care neighborhood and outpatient therapy. We are excited to “Revolutionize Our Care Delivery™” with an array of expanded clinical programs under the leadership of area specialists to better meet the needs of our residents.
